The purpose of the Reach for Home Transitional Housing Program is to provide opportunities and support for Participants to work toward self-sufficiency, independence and permanent housing. Participants work with a Program Manager to help them access resources and services that will help them make necessary changes in their lives. Further, the Program offers Participants the opportunity to develop a good credit history and positive rental history, to gain self-confidence and to become self-sufficient.

Our Mission is to end homelessness in North Sonoma County. Programs to provide opportunities and support for clients to work toward self-sufficiency, independence and permanent housing.

OUR MISSION:

Ending homelessness in Northern Sonoma County.

By engaging with community members who are struggling with or at risk of losing housing, we help transform lives with action plans for their success. Our programs promote self-sufficiency, independence, and permanent housing. Through hope, heart, tenacity, and synergy, we strive for dignity for every member of our community.

Empathy and care drives all we do for each other and our community. Our compassion is grounded in authenticity, core boundaries, and we strive to meet people where they are with an open mind believing every human being has a right to their dignity.

Regardless of the circumstance, we believe change and growth is possible for every person, organization, and community. We have a responsibility to our own self-care in order to continue to offer hope to one another and people we serve.

In spite of choice and barriers, we strive to provide opportunities and equity for our participants. We do what we say and are consistent with our services. We are accountable to one another and our stakeholders.

Synergy

We recognize that our work is complex, and no single person or organization can do this work alone. We work in concert with each other, our participants, and our community partners to end homelessness in Northern Sonoma County.

OUR PURPOSE:

Furthermore, our program offers participants the opportunity to gain self-confidence and to become self-sufficient. We assist participants that live in Windsor, Healdsburg, Cloverdale and Geyserville.

Message from our Chief Executive Officer:

In 2022, Reach for Home continued living our core values of hope, tenacity, synergy, and heart in order to advance our mission to end homelessness in Northern Sonoma County. Our core values were evident in all we accomplished thanks to your support and dedication to our mission.  

The first participants moved into the L & M Village in November and have built a sense of community! They are helping clean the campus, gleaning the property’s amazing fruit, and working on jewelry making, painting and other creative projects.  This in addition to their main purpose: working on their goals and timelines to achieve permanent housing.  In just seven weeks, 93% of participants have increased their income and two have received vouchers for permanent housing. 

Throughout the year partnering and collaborating with others resulted in new successes.  Working with the City of Healdsburg, City of Cloverdale, Sonoma County Behavioral Health and other community partners, we helped several extremely vulnerable, long-time unsheltered residents get the long-term care they needed.  In another effort, Reach for Home coordinated with Sonoma County and the Healdsburg Police Department to help 18 individuals at the Healdsburg Park and Ride get support and assistance.  

Your support makes a huge difference to the people we serve. We are excited to see what the new year brings with you by our side! Reach for Home will continue to build a system to end homelessness by working with donors, community partners and volunteers.  Enclosed are additional highlights of what you also helped achieve.  Thank you so much for your support, partnership, and compassion to provide life-changing care in our community to end homelessness.  We can’t wait to see what we can do together next!
