The purpose of the Reach for Home Transitional Housing Program is to provide opportunities and support for Participants to work toward self-sufficiency, independence and permanent housing. Participants work with a Program Manager to help them access resources and services that will help them make necessary changes in their lives. Further, the Program offers Participants the opportunity to develop a good credit history and positive rental history, to gain self-confidence and to become self-sufficient.

Helping people who are experiencing homelessness is not a seasonal task. Consistent, concerted efforts by people who care make a big difference. At Reach for Home, we collaborate with program participants and community leaders to make positive changes on multiple levels every day. We build trust by doing what we say and keeping our promises. With compassion and respect, we partner with individuals experiencing homelessness to identify and accomplish goals.

Right now, nearly 70 people who have experienced homelessness in Northern Sonoma County are living in Reach for Home housing. This includes 20 Northern Sonoma County residents participating in the new interim housing program at the L & M Village who were chronically homeless - people you probably saw on the street.

By becoming a Sustainer of Hope, you offer long-term solutions to unsheltered community members. Pledged monthly income builds trust with participants by offering consistent staffing, programming and hope.
