The purpose of the Reach for Home Transitional Housing Program is to provide opportunities and support for Participants to work toward self-sufficiency, independence and permanent housing. Participants work with a Program Manager to help them access resources and services that will help them make necessary changes in their lives. Further, the Program offers Participants the opportunity to develop a good credit history and positive rental history, to gain self-confidence and to become self-sufficient.

Northern Sonoma County’s First Interim Housing Program

A partnership between Reach for Home, the City of Healdsburg, and Burbank Housing secured state and county funding in December 2021 to purchase the 22-unit L & M Motel, which will become the first and only interim housing program for chronically homeless individuals in Northern Sonoma County.

Program Description

At the L & M Village, chronically homeless individuals will gain the support and skills needed to transition to permanent housing through individualized case management. Being homeless is traumatizing. For most chronically homeless individuals, it is one trauma among many they have experienced in their lifetimes. The L & M Village will create a safe, therapeutic setting where individuals experiencing homelessness can feel accepted and empowered to gain greater independence.

Program participants (overwhelmingly from Northern Sonoma County) will be selected based on their chronically homeless status as defined by the federal Housing and Urban Development standards. Each individual will stay between one and six months, depending on their needs and progress. Please note that the L & M Village is not a drop-in shelter. Reach for Home will operate the L & M Village with trauma-informed care principles always in the forefront. Staff and volunteers will consider the invasive nature of trauma and strive to promote an environment of healing and recovery rather than practices and services that may inadvertently re-traumatize individuals.

Four case managers will be dedicated to the shelter’s 22 residents. Together, case manager and resident will create their own individualized plan for attaining permanent housing, which can include access to employment services and mentorship programs, goal setting, mental health services, addiction treatment, budgeting, and basic life skills. Case managers’ shifts will be staggered so someone is available seven days a week.

The campus’ beautiful gardens and outdoor areas will lend themselves to therapeutic recovery for residents and are an inviting gathering place for community meals and meetings. The warm and cozy rooms will give participants the calm, restful space they need to focus on recovery. The L & M Village will also offer free laundry facilities and a small dog park.

A facilities technician will keep the property maintained and the environment harmonious. The L & M Village will have a facilities mentoring program so that residents can learn skills and gain employment.

Why is the L & M Village Needed?

Every day, Reach for Home staff supports community members who are at the lowest point in their lives and have nowhere else to turn - the ones who fall through the cracks. They may be living in their cars, in an encampment by the river, or moving from place to place trying their hardest to find a safe spot to sleep for the night. They all have one common need: to live with dignity in a stable environment. To have hope.

There are over 150 unsheltered individuals in Northern Sonoma County. In 2021, Reach for Home conducted an informal survey of 77 unsheltered Northern Sonoma County residents and learned that 64 of those surveyed are interested in working towards gaining permanent housing. They face a number of challenges: housing affordability and availability; moving costs; employment challenges; alcohol and drug dependency; psychiatric and mental health conditions; and physical disability or chronic health problems. 

Improving the quality of life of unsheltered community members isn’t just about a roof over their heads. Sustainable change requires caring case management and wraparound services in a safe environment.
